We participate with the insurance companies listed below. Please bring your card with you for every visit. It is your responsibility to know what visits/tests/procedures are included under your policy. You will be billed directly for non-covered services. We follow the recommendations of the American Academy of Pediatrics and practice current standards of care. We shall advocate on your behalf, but realize that your insurance is a contract between you and your insurance company. Some necessary/requested medical care may not be covered and will be your responsibility.

Additionally, it is often the case that our office provides services that may not be associated with an office visit or may be in addition to an office visit. For this reason, there are fees associated with administrative work and telehealth services. Please be advised that a telephone call addressing any clinical concerns may incur a telehealth fee. Please see our current fee list below:
No copay/deductible payment received at time of visit: $10 surcharge – this covers the additional cost of billing
Missed appointments for physical exams, medication check-up & consults OR cancellations with less than 24 hours’ notice: $100
“NO Show” for all other appointments: $50
Walk-in Fee (billed to insurance): $75
Prescription fee for prescriptions provided not associated with an office visit: $30
Fee for Referrals without office visit: $25
Fee for Referrals expedited: $40
Composition of Letters: $50
Copy of Medical Records $.65/page, plus $20 administrative fee

Medication Authorization/Allergy Forms: $20 1st page, $10 each subsequent page
Comprehensive camp & college forms: $40
